Enrollment & Eligibility

Enrollment Services & Patient Assistance

SVCHC, in collaboration with the Aliados Health consortium, our Certified Enrollment Counselors assist to access quality healthcare, insurance, and essential resources.

Our Certified Enrollment Counselors are here to help individuals and families understand their options and enroll in programs such as Covered California, Medi-Cal, Partnership HealthPlan, and CalFresh.  In addition, we help connect to community resources, including housing, transportation, food assistance, and other supportive services. Although we do not assist directly with Medicare enrollment, we can provide a list of resources and brokers who offer Medicare guidance and assistance.

To schedule an appointment with one of our Certified Enrollment Counselors, please call 707.939.6070.

For questions or to make an appointment, please call 707-939-6070 or via email to eligibility@svchc.org.
