En colaboración con La Luz Center, Redwood Community Health Coalition y Sonoma County Human Services, Sonoma Valley Community Health Center llevará a cabo el Evento de Inscripción Abierta de Covered CA el sábado, 11 de enero de 2020 de 9am-2pm.

Participantes necesitarán tener prueba de: Identificación con foto, Ingresos (Impuestos & talones de cheques recientes), prueba de ciudadanía, números de seguro social /ITIN, y fechas de nacimientos de todos los dependientes en los impuestos.

La fecha de vencimiento de Covered CA es el 31 de enero. Aquellos que se inscriban antes del 15 de enero tendrán cobertura a partir el 1 de febrero.

El periodo de Inscripción Abierta está aquí. Asegúrese de renovar o inscribirse antes del 15 de diciembre, 2019 para comenzar cobertura para el 1 de enero, 2020. Para empezar cobertura el 1 de febrero, 2020 asegúrese de inscribirse antes de del 15 de enero, 2020. El periodo terminara el 31 de enero, pero la cobertura no comenzaría hasta el 1 de marzo, 2020. Tome en cuenta que el estado de California implementara multas para personas sin seguro médico durante el año 2020 en el año 2021. 

Para hacer una cita favor de llamar al 939-6070. Favor de traer pruebas de: Identificaciones Ingresos (impuestos 2019 y talones de cheque), estatus migratorio, número de seguro o ITIN y fechas de nacimiento para  cada persona en sus impuestos

La Mini-Feria de Preparación ante Emergencia tomará lugar en El Centro de Salud Comunitario del Valle de Sonoma el viernes, 25 de octubre de 10am-12pm. Este evento es en colaboración con Oficina de Sheriff del Condado de Sonoma, Nuestra Comunidad y el Departamento de Incendios y Rescate del Valle de Sonoma. Participantes tendrán la oportunidad de aprender sobre preparación ante emergencia y entrar en un sorteo de ganar mini kit de emergencia. Participantes tendrán la oportunidad de hacer preguntas sobre inscripción o cualquier cambio de seguros durante Inscripción Abierta 2020 referente a Covered CA, Medicare y Medi-cal. Sonoma Valley Community Health Fair

The Sonoma Valley Community Health Fair is scheduled for Saturday, March 2 at Altimira Middle School 17805 Arnold Dr. from 10 a.m. to 1 p.m. This year’s organizing committee includes the Sonoma Valley Community Health Center in partnership with Dragones Latinxs, La Luz Center, Parent University, Sonoma Valley Hospital, 4Cs of Sonoma County, the Club at Maxwell, Valley of the Moon Lions Club and Vintage House.

This free event will provide health services and information to the community with an emphasis on the needs of underserved communities. Local and county organizations will participate and offer a wide range of resources and information to participants including health insurance enrollment, emergency preparedness kits, free flu shots, non-metabolic health screenings including body mass index, glucose, blood pressure and vision screenings.

An array of activities, including raffles and entertainment, a kids’ corner, a DJ, Ballet Folklorico Quetzalen, Ohtli Yolilitzli Aztec dance group and Vintage House Ukulele group. Refreshments will also be available.

Emergency Preparedness Fair

This free event will take place on Saturday, May 11, at Sassarini Elementary School, 652 Fifth St. W. from 10 a.m. – 2 p.m.

The Emergency Preparedness Fair will feature presentations from the American Red Cross, Sonoma Valley Fire and Rescue and the Sonoma County Sheriff’s Office. In addition, there will be a variety of informational booths.

Participating agencies will provide a range of information from communication sources, health services, home and renters insurance, community resources, fire-safe landscaping, and financial literacy. There will also be a Fun Zone with engaging kid’s activities organized by The Club Maxwell Village, as well as an interactive healing art activity by Sonoma County artist Martin Zúñiga.

Sonoma Valley Fire and Rescue, and the Sonoma Sheriff’s Office will also have emergency vehicles and mobile units on display.

The event is bilingual and is free. There will be free food from La Escondida Food Truck and families will receive a free emergency kit while supplies last.

Participating agencies include, AAA Sonoma, Allstate, American Red Cross, the Sonoma Ecology Center, Exchange Bank, Farmers Insurance, KSVY, La Luz Center, Pets Lifeline, Redwood Credit Union, Sonoma Valley Chamber of Commerce, Sonoma County 211, Sonoma County Sheriff’s Office, Sonoma County Human Services, Sonoma Valley Hospital, Sonoma Valley Fire and Rescue and Vintage House Senior Center.

Back to School Fair

Students and parents prepare for a healthy and successful academic year at the annual Back to School Health Fair on Saturday, August 10, from 10 a.m. to 2 p.m. Please check back for additional details regarding event location.

Students and parents prepare for a healthy and successful academic year at the annual Back to School Health Fair on Saturday, August 10, from 10 a.m. to 1 p.m at Altamira Middle School.

The free event is presented by the Sonoma Valley Community Health Center in partnership with Sonoma Valley Hospital, La Luz Center, Dragones Latinxs, The Club Maxwell Village, 4C’s of Sonoma County, Valley of the Moon Lions Club and FISH of Sonoma Valley.

The day covers a range of health services and activities, including: fluoride treatments; TDAP vaccines; health education and healthy snacks; vision and posture screenings; Fun Zone, educational presentations; complimentary food and school supplies giveaways. The fair is bilingual and is open to all Sonoma Valley students.

Attendance and completion of the Back to School Health Fair passport are required in order to receive a free backpack filled with age appropriate school supplies. The TDAP vaccine will be available for entering seventh graders. No appointments are necessary. A parent must be present to sign the consent forms for all screenings and vaccines.

The school supply drive continues through August 2. Donations of the school supplies will be accepted at the Sonoma Valley Community Health Center, 19270 Sonoma Hwy or the Sonoma Staples store at 977 W Napa St.
